I had already generated a full report example that involved answering ALL of the questions. I went in and changed one answer in one section, and exported the report again. Some of the steps came out in the report as not having been completed.
I was able to export a full report again by clicking through and saving each step again. I haven't tried to repeat this error yet, but I suspect it happens when you don't enter an area, it may not get captured in the report?
If repeatable this should be addressed.

I just fixed a bug where pdf generation doesn't work if you imported a project, not sure if that's what you were running into here.
With the exception of importing, the only other time data gets saved to the pdf report is when the step is saved (so your suspicions were correct), however, the application should still have all of the steps from when you answered the questions earlier (unless you imported, but that's fixed now).
OK, that makes sense then. So for now, I could note in the instructions that you need to go through a step to get it added in the report for sure. Or have a troubleshooting section that mentions this. Thanks!
That probably wouldn't hurt just to be safe. With the exception of importing (fixed), this shouldn't ever happen. There isn't a way to enter data for a step without going through it and it should save on every action.
