Fix document regarding patent policy, remove unnecessary rule for DNOTE

Currently the document for Patent Policy looks like this:

I didn't distinguish rec-track document from the others, will do when implementing #1470, splitting both the rule and the document.

I updated the doc for patpolreq (see 48e0c6c) to fix some markup issues and to make the items more consistent.
